My name is Pete Collins and I am the Person Centred Planning Co-ordinator for Southampton.
It is my job to make sure that people with learning disabilities and those that provide support find out about person centred planning and have the opportunity to plan their life the way they want it.
I have worked with people with learning disabilities in Hampshire & Southampton for 20 years in a residential home, day services, care management and now in this role for the last 4 years.
I work mainly with adults but also give some support to my colleague Phil with his work in transition.

Phil Orchard
My name is Phil Orchard and I am a Person Centred Planning Transition Worker in Southampton.
This means that I help young people who are 14 years old or older to plan their lives and their future.
The other part of my work is to lead the Annual Reviews in Special Schools when pupils are in Years 9, 10 and 11
